Medical
We offer two medical plans with Humana Inc. The copayments for physician services and Rx are the same for
both plans. The major difference between the plans is the deductible. You can view participating providers by accessing their website at www.humana.com and select network National POS / Open Access Plus.

Short-Term and Long-Term Disability
We offer short-term and long-term disability through
Mutual of Omaha to replace your income if you are off
work due to a non-work related injury.
Voluntary Life Insurance
We have a voluntary life insurance program with Mutual of
Omaha. An employee can enroll for up to 5 times their annual salary but
the guaranteed issue amounts are
$75,000 for employee and $37,000 for spouse. Anything over these amounts would have to go through underwriting to be approved. The rates are based on age bands of every five years and the coverage does reduce
when a participant reaches age 65 and a spouse at age 60.
